National Lite:

  • Printed copy of your players AAU card

  • HPVBA Medical Release Form

Elite:

  • Printed copy of your players AAU card
  • Copy of Birth Certificate (please black out any SSN’s)
  • HPVBA Medical Release Form
  • Online completion of JVA Insurance Waivers

Premier:

  • Printed copy of your players AAU card
  • Online completion of USAV card (we will send out an invitation)
  • Copy of Birth Certificate (please black out any SSN’s)
  • HPVBA Medical Release Form
  • Online completion of JVA Insurance Waivers

AAU Registration

(ALL TEAMS)

  • Follow the registration link
  • Register "My Child" as an "Athlete" 
  • Fill out all information requested
  • Be sure to select "High Performance Volleyball Academy" (club code: W378YW)

USAV Registration

(NIKE TEAMS ONLY)

  • Follow the Parent/Athlete link
  • Select High Performance Volleyball Academy
  • Ensure you register for a whole season, not a single event
  • If you do not have a SportsEngine account, you will be prompted to make one
